No, the full title is as Sanfan said. "Country Roads" is the 'bastardized' title the girls give it in the movie. http://en.wikipedia.org/wiki/John_Denver. It seems to get called just "Country Roads" a lot of the time though, perhaps people forget the full title?

Or it's just easier to remember. As an aside, the Whisper of the Heart OST refers to it as "Country Road".

The opening song is "Take Me Home, Country Roads," performed by Olivia Newton-John. It is not on the soundtrack CD (but easily available on many others).
The ending is "Country Road," performed by HONNA Yôko. It is on the soundtrack CD.
They're two different songs with different titles, lyrics and arrangements, but sharing the same tune (the final version of "Country Road" is more than just a translation; the lyrics are quite different and relate to Shizuku's life).
